Pay Range: $59,000 - $ 65,000

POSITION SUMMARY

The Confederacy of Treaty Six First Nations (CT6FN) is a non-profit Political and Treaty Organization (PTO) that has been mandated by the Chiefs and Elders of Treaty No.6 to advocate, protect, and enhance Treaty rights by monitoring legislation and policies that affect Treaty rights.

The Confederacy of Treaty Six First Nations (CT6FN) is seeking a dedicated and skilled Patient Navigator to join our Health Team within the Policy and Planning Group. This is an opportunity to work closely with the Member Nations of Treaty No. 6, contributing to the advancement of health-related policies and initiatives that align with the strategic objectives of the Confederacy.

Responsibilities

  • Assist clients in understanding and accessing health services, including primary care and acute care, specialists, diagnostics, mental health support, and traditional healing.

  • Support clients in preparing for and attending appointments, including arranging transportation or accompaniment where appropriate.

  • Act as a liaison between clients and health care providers to ensure clear communication and informed decision-making.

  • Help clients navigate medical referrals, discharge instructions, and follow-up care.

  • Advocate for clients to receive equitable and respectful treatment within the health care system.

  • Support culturally appropriate care by working with traditional knowledge holders, Elders, and cultural support when requested.

  • Identify and address systemic barriers (e.g., racism in health care, access to funding or services) that may impact client health outcomes.

  • Link clients to internal and external resources, including housing support, income assistance, mental health services, addiction support, and food security programs.

  • Maintain up-to-date knowledge of Indigenous and non-Indigenous health and social service programs.

  • Build and maintain relationships with local hospitals, clinics, community agencies, and Indigenous health networks.

 

Educations, Qualifications, and Work Experience

  • Post-secondary diploma or degree in a related filed, i.e., Social Work, First Nations governance, humanities (English, history, psychology, political science).

  • Equivalencies may be considered based on one year of experience for one year of education or one year of education for one year of experience. 

  • Minimum 2 years of experience in advocacy, case management, or health-related work.

  • Knowledge of Indigenous health perspectives, traditional healing, and the impact of colonialism on Indigenous health.

  • Strong understanding of patient rights, informed consent, and health care systems.

  • Excellent communication, conflict resolution, and listening skills.

  • Trauma-informed, non-judgmental, and respectful approach to client support.

  • Cultural humility and ability to work effectively with Indigenous clients and families.

  • Ability to work independently and collaboratively with health care professionals.

  • Ability to work independently, prioritize tasks, and adapt to changing priorities in a dynamic environment.

  • Strong communication skills, including active listening, verbal articulation, and written correspondence.

  • Proficiency in using computer applications and databases for documentation and record-keeping.

  • The ability to communicate in one of the affiliated First Nations of the Confederacy (Cree, Nakoda, Dene, Saulteaux) is a definite asset.

  • Preference will be give to Treaty Six First Nation Members.
